Teresa Dominguez Killed in a Forklift Work Accident at H-E-B Warehouse in San Antonio, Texas

San Antonio, Texas — A 27-year-old employee at an H-E-B warehouse in San Antonio died Friday after sustaining injuries that may have involved a forklift.

Police were called to the warehouse located in the 5700 block of Rittiman Road on Friday morning. The employee was operating a forklift when she stopped and began showing signs of distress. Another worker noticed the situation and contacted emergency medical services.

Teresa Dominguez was transported to a hospital, where she was later pronounced dead.

Dominguez suffered a blunt-force injury to her lower left leg. A police offense report notes a “crush injury” to the same area, and preliminary findings indicate a forklift may have caused the injury.

Fatal workplace accidents must be investigated for safety failures

Young, hardworking, and trying to make a living, only to never return home to her family. This should never be. One question we hear again and again from families after tragedies like this is: How could this happen? This is a question that deserves clear, honest answers.

Understanding what happened in those final moments begins with looking at the environment Ms. Dominguez worked in: the training provided, the supervision in place, and the safety measures meant to protect her. Knowing whether the forklift operator was adequately trained and equipped with safety protocols for operating such machinery is important.

Common factors in these accidents often include gaps in training or unclear procedures for those responsible for operating heavy equipment. Those in charge are responsible for ensuring their workers receive proper instruction and have access to the tools and safety measures needed to perform their duties safely.

A thorough investigation by trained professionals will reveal whether any gaps in safety protocols or possible equipment malfunctions contributed to this accident. If it is uncovered that this incident could have been prevented, that there was a safety failure along the way, those responsible should be held accountable.

I hope Ms. Dominguez’s family finds the answers they deserve.
