Worker Carlos Medina, of El Paso, Texas, Dies in a Wisconsin City Water Tank Accident in Blair, Wisconsin

Blair, Wis.— A Texas man died when he got trapped cleaning a municipal water tank in Blair, Wisconsin.

According to reports, Carlos Medina, 30, of El Paso, Texas, was cleaning the tank using a 90-foot suction tube when he told coworkers he felt cold. He continued working when he then became entangled in the suction tube.

First responders and two workers made efforts to retrieve Medina from the tank, where he was found motionless and submerged underwater. He was declared deceased on the scene.

No further details have been provided.

Thoughts on responsibility following a fatal workplace incident

The family of Mr. Medina deserves answers. They deserve to know if precautions were taken to prevent this tragedy from happening. They deserve to know why greater actions were not taken by the employer to prevent the death of their loved one.

If negligent actions contributed to the incident, it is vital that those responsible are held accountable. Safety should always be a top priority in any workplace, and failure to uphold safety standards must not go unpunished.

Our hope is the family of the deceased receives the answers and justice they deserve.
